SF-2801 Application for Retirement (Must be Completed)
If you are a current Federal or Postal Service employee covered by the Civil Service Retirement System and you wish to apply for retirement with an immediate annuity (annuity commencing within one month after the date of separation on which title to the annuity is based), this package is for you! W-4P Withholding Certificate for Pension or Annuity Payments
Form W-4P is for U.S. citizens, resident aliens, or their estates who are recipients of pensions, annuities (including commercial annuities),
and certain other deferred compensation. Use Form W-4P to tell payers the correct amount of federal income tax to withhold from your payment(s). You also may use Form W-4P to choose (a) not to have any federal income tax withheld from the payment (except for eligible rollover distributions or for payments to U.S. citizens to be delivered outside the United States or in possessions) or (b) to have an additional amount of tax withheld. When you first become eligible for FEGLI you may (1) elect Basic and any or all of the options,
(2) elect Basic but waive all of the options, or (3) waive all life insurance coverage. If you are changing
a previous election, see the back of Part 3 - Employee Copy.
SF-2818 Continuation of Life Insurance (complete if you have Life Insurance)
Complete this form :
- When you retire, or
- When you are receiving compensation payments from the (Office of Workers' Compensation Programs Department of Labor) and your FEGLI coverage as employee ends.

Information for Retirees and Their Families, Federal Employees' Group Life Insurance
This pamphlet contains important information for retired Federal employees about rights and benefits as an annuitant under the Federal Employees' Group Life Insurance Program. VIEW AND PRINT
Tip: Once your forms are received by OPM, your assigned Retirement Counselor will review your application for completeness, contact you to confirm receipt, and discuss any issues or questions you may have.
OPM issues monthly interim payments of approximately 60 to 80 percent of the annuity computation within 6 to 8 weeks of retirement. These payments continue until final adjudication is complete. Because OPM is backlogged in the processing of retirement cases for all of their customers - you may remain in interim pay status from 4 to 6 months. Once your annuity has been finalized your annuity payments will be set to 100% and OPM will reimburse you for any funds that are due you. If you do not receive an interim payment within 8 weeks, call OPM toll-free 1-888-767-6738. After your retirement date, OPM will be able to provide you guidance on any additional retirement questions you might have.
